Intern Sub Request

This form is only to be submitted by Elementary and Jr. High Head Custodians.

Do NOT request a sub for the day of an absence or less than 24 hours in advance of an absence. In that case, call Boyd or Amanda on their cell phones.

Substitute request is not guaranteed until you receive an email confirmation.

You are still responsible to enter all leave on Employee Online and to notify your immediate supervisor (Principal).
